* Final approval by the Building Official is required prior to occupancy. *Work shall not proceed until the inspector has approved the stages of construction. *Surface storm water shall be diverted from the building site and shall not drain onto adjacent properties. * I hereby acknowledge that I have read this Permit/Application, that the information given is correct: that I am the owner or the duly authorized agent of the owner; that plans submitted herewith are in compliance with all applicable city, county and state laws and that all construction will proceed in accordance with said laws. This permit shall expire if work is not commenced with 180 days or if the work is suspended for a period of 180 days. Once work has started (passed a required inspection) one inspection per year must be completed to keep active or it will formally expire and become null and void. * By leaving the contractor information section blank, I hereby certify further that contractors (general or subcontractors) will not be hired to perform any work in association with this permit. I also certify that if I do choose to hire a contractor (general or subcontractor) I will only hire those contractors that are licensed by the state of Washington.

PERMIT CONDITIONS: FIRE: 1. Provide and install fire extingushers per code. 2 Any changes to the existing occupancy classification will require a permit. A-3 is the current occupancy classification. 3. Front and rear doors require signage that says THIS DOOR TO REMAIN UNLOCKEDWHEN BUILDING IS OCCUPIED. 4. Any cooking on the kitchen stove that produces grease laden vapors will require a Type I hood and ducting and a UL300 hood suppression system. 5. Annual inspections will be conducted by a qualified City of Puyallup employee and to be accompanied by a CPFR Deputy Fire Marshal. BUILDING: 1) Any owner or authorized agent who intends to construct, enlarge, alter, repair, move, demolish, or change the occupancy of a building or structure, or to erect, install, enlarge, alter, repair, remove, convert or replace any electrical, gas, mechanical or plumbing system, the installation of which is regulated by this code, or to cause any such work to be done, shall first make application to the building official and obtain the required permit. 2)Post all rooms and spaces with Occupant Load signage per occupant loads calculated and on approved plans.
