Once work has started (passed a required inspection) one inspection per year must be completed to keep active or it will formally expire and become null and void. * By leaving the contractor information section blank, I hereby certify further that contractors (general or subcontractors) will not be hired to perform any work in association with this permit. I also certify that if I do choose to hire a contractor (general or subcontractor) I will only hire those contractors that are licensed by the state of Washington. DEMOLITION SHALL MEET ALL REQUIREMENTS OF WASHINGTON STATE AND PSCAA.

: 1. I agree to call the Engineering inspector at 253-405-1707 to inspect the cut and cap of the sanitary sidesewer at property line. The existing sidesewer shall not be re-used. 2. I agree to call the inspection line at 253-445-4795 or 253-445-5092 to schedule a building final inspection. Enter action code 9800 when prompted for Building Inspection Final. 3. I agree to call the Utility Notification Center at 1-800-424-5555 before beginning any excavation. Call before you dig, it's the law. The sewer and water service to this building will be cut and capped as required by the City of Puyallup. This site now has 6 water & 6 sewer fixture unit credits. I hereby acknowledge that I have read and understand the contents of this permit and I hereby state that the information I have supplied is true and correct. The Applicant shall defend, indemnify, and hold the City, its officers, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the Applicant or on the Applicant's behalf out of issuance of this permit.
Pipeline Notfication Rcw 19.122.033 Resolved Engineering Division Standard Conditions
: Effective January 1, 2013 RCW 19.122 Before conducting any construction or excavation within 100 feet of a right-of-way or utility easement containing a transmission pipeline a person must notify the pipeline companies of the scheduled excavation through the one-number locator service 811. Notification must occur in a window of not less than 2 business days but not more than 10 business days before beginning the excavation. If a transmission pipeline company is notified that excavation work will occur near a pipeline, a representative of the company must consult with the excavator on-site prior to excavation.
