* Final approval by the Building Official is required prior to occupancy. *Work shall not proceed until the inspector has approved the stages of construction. *Surface storm water shall be diverted from the building site and shall not drain onto adjacent properties. * I hereby acknowledge that I have read this Permit/Application, that the information given is correct: that I am the owner or the duly authorized agent of the owner; that plans submitted herewith are in compliance with all applicable city, county and state laws and that all construction will proceed in accordance with said laws. This permit shall expire if work is not commenced with 180 days or if the work is suspended for a period of 180 days. Once work has started (passed a required inspection) one inspection per year must be completed to keep active or it will formally expire and become null and void. Permits expire 2 years from issuance. * By leaving the contractor information section blank, I hereby certify further that contractors (general or subcontractors) will not be hired to perform any work in association with this permit. I also certify that if I do choose to hire a contractor (general or subcontractor) I will only hire those contractors that are licensed by the state of Washington.

PERMIT CONDITIONS: FIRE: 1. ANY MODIFICATION TO THE FIRE ALARM OR SPRINKLER SYSTEM SHALL REQUIRE A SEPERATE PERMIT. 2. PUYALLUP MUNICIPAL CODE REQUIRES "TOTAL COVERAGE" PER NFPA 72. 3. PUYALLUP MUNICIPAL CODE REQUIRES UL CERTIFICATION ON NEW FIRE ALARM SYSTEMS. 4. SEISMIC CALCULATIONS SHALL BE REQUIRED ON THE NEW AND EXISTING FIRE SPRINKLER SYSTEM. 5. AN EMERGENCY EGRESS LIGHTING TEST SHALL BE REQUIRED PRIOR TO TCO OR FINAL OCCUPANCY. THE TEST SHALL BE CONDUCTED AFTER ALL EXCERCISE EQUIPTMENT IS INSTALLED. COMPLY WITH IFC 2012 EDITION SECTION 1006. REQUIRED INSPECTIONS: 1 IVR-2252 EMERGENCY EGRESS LIGHTING TEST 2 IVR-9200 FIRE FINAL BUILDING PREMIT INSPECTION ENGINEERING: Please note on page P1.2 note 2 of Bass Bid that "connection to or construction of this line is not permitted under this permit". Prior to occupancy the water service being provided to this tenent will be protected by the installation of an Reduced Pressure Backflow Assembly.
Pipeline Notfication Rcw 19.122.033 Resolved Standard Conditions
Engineering Division
Effective January 1, 2013 RCW 19.122 Before conducting any construction or excavation within 100 feet of a right-of-way or utility easement containing a transmission pipeline a person must notify the pipeline companies of the scheduled excavation through the one-number locator service 811. Notification must occur in a window of not less than 2 business days but not more than 10 business days before beginning the excavation. If a transmission pipeline company is notified that excavation work will occur near a pipeline, a representative of the company must consult with the excavator on-site prior to excavation.
