* Final approval by the Building Official is required prior to use or occupancy. * Work shall not proceed until the inspector has approved the stages of construction. * Surface storm water shall be diverted from the building site and shall not drain onto adjacent properties. * I hereby acknowledge that I have read this Permit/Application, that the information given is correct; that I am the owner or the duly authorized agent of the owner; that plans submitted herewith are in compliance with all applicable city, county and state laws and that all construction will proceed in accordance with said laws. This permit shall expire if work is not commenced with 180 days or if the work is suspended for a period of 180 days. Permits expire two years from issuance. * By leaving the contractor information section blank, I hereby certify further that contractors (general or subcontractors) will not be hired to perform any work in association with this permit. I also certify that if I do choose to hire a contractor (general or subcontractor) I will only hire those contractors that are licensed by the State of Washington. * TO SCHEDULE INSPECTIONS, CHECK PERMIT STATUS OR TO MAKE PAYMENTS ON THE CITY WEB SITE VISIT https://secure.puyallupwa.gov Conditions of approval: 1) Letters of acknowledgement from the Pierce County Department of Emergency Management and/or the Washington State Emergency Management Division regarding coordination of the on-site emergency notification system with one or both of these agencies. 2) Prior to occupancy of the building: a. Connection to the alert and warning emergency system, Outdoor Warning System. The applicant shall provide documented proof of their on-site emergency alert system and that it has been successfully connected to and integrated with the regional laharalert notification system. The city may consider a phased occupancy approach for limited activity (e.g. stocking, security of product within the structure by limited security staffing, etc.) prior to full activation of the emergency alert system upon written approval by the City Director of Development & Permitting Services, City Building Official and City Emergency Manager. b. Adoption of an emergency evacuation plan from the site which is coordinated with the City of Puyallup adopted emergency evacuation plan to provide a route to from the site to the City's adopted evacuation routes. c. Provide an adopted plan which designates an on-site emergency coordinator to ensure the evacuation plans are maintained and updated. Annual updates to the emergency evacuation plan to maintain consistency with on-going (active) updates to the City of Puyallup emergency management evacuation planning efforts; annual update confirmation shared in writing with the City of Puyallup Emergency Management Director. Pipeline Notfication Rcw 19.122.033 Resolved Standard Conditions

Effective January 1, 2013 RCW 19.122 Before conducting any construction or excavation within 100 feet of a right-of-way or utility easement containing a transmission pipeline a person must notify the pipeline companies of the scheduled excavation through the one-number locator service 811. Notification must occur in a window of not less than 2 business days but not more than 10 business days before beginning the excavation. If a transmission pipeline company is notified that excavation work will occur near a pipeline, a representative of the company must consult with the excavator on-site prior to excavation.
