: PERMIT CONDITIONS: * THE APPLICANT IS RESPONSIBLE TO CALL THE INSPECTION LINE AT 253-445-4795 OR 253-445-5092 TO SCHEDULE INSPECTIONS. UTILITY INSPECTIONS ARE REQUIRED PRIOR TO BACKFILLING. * For Engineering questions arising in the field, please contact the Engineering Inspector at 253-435-1707. * No work shall be done in or on the public right of way without a licensed and bonded contractor first obtaining a right of way permit. 1. Adhere to approved Traffic Control Plan. 2. Any curb, gutter or sidewalk broken now or during construction shall be removed and replaced per City Standards. 3. The applicant is responsible for keeping the existing right of way free of debris and dirt. Sediment control and erosion procedures shall be implemented to prevent off site damage. 3. Driveway approach must be a minimum 15 feet wide and max 30 feet wide. Total width of all driveways shall not exceed 50% of property frontage, per City Standards. 4. Curb, gutter, sidewalk and approach shall be installed per City Standards. 5. 6. The applicant is responsible to cut and cap the sanitary sidesewer at property line. The existing sidesewer shall not be re-used. 7. The applicant is responsible to decommision the existing septic system per P.C.H.D requirements. A decommissioning certificate is required to be filed with P.C.H.D. and a copy submitted to the City of Puyallup. 8. The applicant is responsible to decommision the existing private well per P.C.H.D. requirements. A decommissioning certificate is required to be filed with P.C.H.D. and a copy submitted to the City of Puyallup.. 9. Installation of an approved backflow assembly is required.
