* The dumpster/container shall not block the traffic lane. There must be sufficient space for emergency vehicles to pass (Fire Department has the right to deny any location). Only upon special provisions will the City allow blocking traffic lanes. * The dumpster/container shall not be placed within fifty (50) feet from an intersection or alley in order to maintain sight distance. * The dumpster/container shall not be placed within ten (15) feet of a fire hydrant, within any 'fire lane', within a 'no parking zone', within five (5) feet of a neighboring curb cut. * The permit applicant is responsible for all snow and debris removal in the area surrounding the dumpster or container. * Applicant assumes all liability and holds harmless the City against all claims, damages, suits, penalties, fines and liability for injury or death to persons or loss or damage to property related to the placement of a dumpster/container in the City Right-of-Way. * Applicant shall display permit in clear view on or near the permitted dumpster or container. * Pursuant to PMC 6.12.030 all dumpsters placed withing the City shall be owned and maintained by DM Disposal.

1. I agree to call the inspection line at 253-445-4795 OR 253-445-5092 to schedule inspections. Enter action code 6910 when prompted. 2. I agree to have the approved permit on site at all times. 3. UTILITY INSPECTIONS ARE REQUIRED PRIOR TO BACKFILLING. 4. For Engineering questions arising in the field, please contact the Engineering Inspector at 253-405-1707. 5. No work shall be done in or on the public right of way without a licensed and bonded contractor first obtaining a right of way permit. 6. I agree to keep the existing right of way free of debris and dirt. 7. I agree to call the Utility Notification Center at 1-800-424-5555 before beginning any excavation. Call before you dig, it's the law. 8. I agree to adhere to the approved Traffic Control Plan. 9. I agree to perform all work in accordance with City's Standards. Any curb, gutter or sidewalk broken now or during construction shall be removed and replaced. 10. I hereby acknowledge that I have read and understand the contents of this permit and I hereby state that the information I have supplied is true and correct. The Applicant shall defend, indemnify, and hold the City, its officers, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the Applicant or on the Applicant's behalf out of issuance of this permit.
