: 1. I agree to call the inspection line at 253-445-4795 OR 253-445-5092 to schedule inspections. Enter action code 6910 when prompted. 2. I agree to have the approved permit on site at all times. 3. UTILITY INSPECTIONS ARE REQUIRED PRIOR TO BACKFILLING. 4. For Engineering questions arising in the field, please contact the Engineering Inspector at 253-405-1707. 5. No work shall be done in or on the public right of way without a licensed and bonded contractor first obtaining a right of way permit. 6. I agree to keep the existing right of way free of debris and dirt. 7. I agree to call the Utility Notification Center at 1-800-424-5555 before beginning any excavation. Call before you dig, it's the law. 8. I agree to adhere to the approved Traffic Control Plan. 9. I agree to perform all work in accordance with City's Standards. Any curb, gutter or sidewalk broken now or during construction shall be removed and replaced. 10. I hereby acknowledge that I have read and understand the contents of this permit and I hereby state that the information I have supplied is true and correct. The Applicant shall defend, indemnify, and hold the City, its officers, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the Applicant or on the Applicant's behalf out of issuance of this permit.
