WADOT HAS NOTED ON THEIR APPROVAL THAT SPECIAL PROVISION #6 REQUIRES NOTIFICATION FOR FINAL INSPECTION OF THIS PROJECT AND SPECIAL PROVISIONS #50 AND #51 HAVE REQUIREMENTS THT MUST BE COMPLETED PRIOR TO WORK BEGINNING.

PRIVATE PERMIT CONDITIONS: Follow approved TCPs (both City and WADOT) all work is to be done on backside of curb as shown in approved plans. *** PERMIT IS REQUIRED TO BE VISIBLE AT JOB SITE *** 1. CALL 253-841-5577 24hrs PRIOR TO STARTING WORK TO OPEN PERMIT. 2. CALL 253-405-1707 TO NOTIFY INSPECTOR OF SPECIFIC PROJECT SCHEDULING. 2. ADHERE TO APPROVED TRAFFIC CONTROL PLAN. 3. DO NOT PARK VEHICLES ON CITY SIDEWALK. 4. DO NOT OPEN CUT WITHOUT PRIOR APPROVAL. 5. ANY CURB, GUTTER OR SIDEWALK BROKEN DURING CONSTRUCTION SHALL BE REMOVED AND REPLACED. 6. CALL 253-841-5577 TO CLOSE PERMIT WHEN WORK IS COMPLETE. 7. I hereby acknowledge that I have read and understand the contents of this permit and I hereby state that the information I have supplied is true and correct. The Applicant shall defend, indemnify, and hold the City, its officers, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the Applicant or on the Applicant's behalf out of issuance of this permit.
