DO NOT PARK VEHICLES ON CITY SIDEWALK. 5. DO NOT OPEN CUT WITHOUT PRIOR APPROVAL. 6. ANY CURB, GUTTER OR SIDEWALK DAMAGED SHALL BE REMOVED AND REPLACED. 7. CALL 253-841-5577 TO CLOSE PERMIT WHEN WORK IS COMPLETE. 8. I hereby acknowledge that I have read and understand the contents of this permit and I hereby state that the information I have supplied is true and correct.
