Marcus Sedlickas will be the inspector for this work. 1. CALL 253-841-5577 24hrs PRIOR TO STARTING WORK TO OPEN PERMIT. 2. CALL 253-405-1707 TO NOTIFY INSPECTOR OF SPECIFIC PROJECT SCHEDULING. 3. ADHERE TO APPROVED TRAFFIC CONTROL PLAN. 4. DO NOT PARK VEHICLES ON CITY SIDEWALK. 5. DO NOT OPEN CUT WITHOUT PRIOR APPROVAL. 6. ANY CURB, GUTTER OR SIDEWALK DAMAGED SHALL BE REMOVED AND REPLACED. 7. CALL 253-841-5577 TO CLOSE PERMIT WHEN WORK IS COMPLETE. 8. I hereby acknowledge that I have read and understand the contents of this permit and I hereby state that the information I have supplied is true and correct. The Applicant shall defend, indemnify, and hold the City, its officers, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the Applicant or on the Applicant's behalf out of issuance of this permit. RCW 19.27 requires the city to provide notification to “transmission pipeline companies” of proposed construction within 100’ of a pipeline easement or right-of-way prior to construction activity being permitted or require consultation between the person proposing the construction activity and the transmission pipeline company as a condition of receiving the permit.
