JOB DESCRIPTION: Trench 16' in the row to place 1-4" PVC and place new cable in existing PVC. Overset existing telco pedestal with a 264TA handhole and overset on existing pedestal with a Bigshot pedestal. We will remove and replace 3 5' x 8' sidewalk panels.

All new pedestals will be concealed with landscaping. A portion of the work to be done at night, see approved TCPs. Night work required in work zone up to 185' west of intersection of S Meridian and 43rd Ave SW Call Comm Center day of closure at 253-841-5431. 1. CALL 253-841-5577 24hrs PRIOR TO STARTING WORK TO OPEN PERMIT. 2. CALL 253-405-1707 TO NOTIFY INSPECTOR OF SPECIFIC PROJECT SCHEDULING. 3. ADHERE TO APPROVED TRAFFIC CONTROL PLAN. 4. DO NOT PARK VEHICLES ON CITY SIDEWALK. 5. DO NOT OPEN CUT WITHOUT PRIOR APPROVAL. 6. ANY CURB, GUTTER OR SIDEWALK DAMAGED SHALL BE REMOVED AND REPLACED. 7. CALL 253-841-5577 TO CLOSE PERMIT WHEN WORK IS COMPLETE. 8. RCW 19.122 requires all owners of underground facilities to notify pipeline companies of scheduled excavations through the one-number locator service if proposed excavation is within 100 feet. Notification must occur in a window of not less than 2 business days but not more than 10 business days before beginning the excavation. If a transmission pipeline company is notified that excavation work will occur near a pipeline, a representative of the company must consult with the excavator on-site prior to excavation. 9. I hereby acknowledge that I have read and understand the contents of this permit and I hereby state that the information I have supplied is true and correct. 10. The Applicant shall defend, indemnify, and hold the City, its officers, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the Applicant or on the Applicant's behalf out of issuance of this permit.
