JOB DESCRIPTION: From HH 3511 place 107' of new fiber in existing duct to STA 1+107 at STA 1+07 place a new HH overset on existing duct, then pull 73' of new fiber in existing duct to HH 3484.

*** PERMIT IS REQUIRED TO BE VISIBLE AT JOB SITE *** Work between the hours of 9am & 3pm Maintain minimum 11' lane widths in each direction Spacing between cones - 25' See taper length formula on TCP ** If impact to traffic is severe, as determined by the City inspector, work will be done at night between the hours of 9pm & 5 am. ** 1. CALL 253-841-5577 24hrs PRIOR TO STARTING WORK TO OPEN PERMIT. 2. CALL 253-405-1707 TO NOTIFY INSPECTOR OF SPECIFIC PROJECT SCHEDULING. 3. ADHERE TO APPROVED TRAFFIC CONTROL PLAN. 4. DO NOT PARK VEHICLES ON CITY SIDEWALK. 5. DO NOT OPEN CUT WITHOUT PRIOR APPROVAL. 6. ANY CURB, GUTTER OR SIDEWALK DAMAGED SHALL BE REMOVED AND REPLACED. 7. CALL 253-841-5577 TO CLOSE PERMIT WHEN WORK IS COMPLETE. 8. RCW 19.122 requires all owners of underground facilities to notify pipeline companies of scheduled excavations through the one-number locator service if proposed excavation is within 100 feet. Notification must occur in a window of not less than 2 business days but not more than 10 business days before beginning the excavation. If a transmission pipeline company is notified that excavation work will occur near a pipeline, a representative of the company must consult with the excavator on-site prior to excavation. 9. The Applicant shall defend, indemnify, and hold the City, its officers, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the Applicant or on the Applicant's behalf out of issuance of this permit.
