CALL 253-841-5577 TO CLOSE PERMIT WHEN WORK IS COMPLETE. 8. RCW 19.122 requires all owners of underground facilities to notify pipeline companies of scheduled excavations through the one-number locator service if proposed excavation is within 100 feet. Notification must occur in a window of not less than 2 business days but not more than 10 business days before beginning the excavation. If a transmission pipeline company is notified that excavation work will occur near a pipeline, a representative of the company must consult with the excavator on-site prior to excavation. 9. I hereby acknowledge that I have read and understand the contents of this permit and I hereby state that the information I have supplied is true and correct. 10. The Applicant shall defend, indemnify, and hold the City, its officers, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the Applicant or on the Applicant's behalf out of issuance of this permit. ** Where a curb exists, the lateral offset for all vertical obstructions shall be a minimum of 1.5' measured from the face of the obstruction pole to the face of curb. ** Where no curb exists, the lateral offset for all vertical obstructions shall be a minimum of 4' measured from the face of the obstruction to the edge of pavement.
Traffic Control Device Requirements Resolved Engineering Division Standard Conditions
: 1. Traffic Control devices shall be retroreflective drums. 2. Taper length equals width of offset times speed limit squared divided by 60. L = WS² /60 3. Sign spacing shall be 100 feet when speed limit is 30 MPH or less. 4. Sign spacing shall be 350 feet when speed limit is greater than 30 MPH. 5. Distance between signs is mandatory unless field conditions determine otherwise. 6. Call Puyallup Comm Center at 253-841-5431 the day of closure. 6.1 Closure for weekdays only. 7. Flaggers shall remain in radio contact at all times. 8. Distance between cones equals speed limit in units of feet.
