: Follow approved Traffic Control Plan (TCP) Work between the night hours of 9PM-5AM Sunday through Thursday. Call Comm Center day(s) of lane closure at 253-841-5431 FOR ALL TRAFFIC CONTROL PLANS: * CALL INSPECTOR TO COORDINATE AT LEAST 1 WEEK IN ADVANCE AT (253)405-1707 * SPOTTER MUST BE AVAILABLE TO ASSIST WITH PEDESTRIANS AS NEEDED. * COMPLETE STREET CLOSURES MAY BE NO LONGER THAN 3 MINUTES AT A TIME. **TCP'S 7, 10, 11 AND 13: TRANSITION POINTS MUST UTILIZE CHANNELIZING DEVICES TO DEFINE TRAVEL LANES. **TCP'S 21 AND 22: PERMIT APPROVAL IS ONLY FOR WORK IN THE CITY OF PUYALLUP RIGHTOF WAY. COORDINATION WITH BNSF WILL BE REQUIRED FOR WORK IN OR OVER THE BNSF RAILS AND PROPERTY. FOR ALL TRAFFIC CONTROL PLANS UTILIZING POLICE OFFICER: * TRAFFIC CONTROL PLANS THAT REQUIRE THE USE OF A POLICE OFFICER SHALL UTILIZE PUYALLUP OFFICERS. CONTACT CHIEF SCOTT ENGLE AT (253)435-3659. 24 HRS ADVANCE NOTICE IS REQUIRED. A. Distance between cones equals speed limit in unit feet. B. Taper length equals width of offset times speed limit squared divided by 60. L = WS² /60 - W= lane width S = speed limit C. Sign spacing shall be 100 feet when speed limit is 30 MPH or less D. Sign spacing shall be 350 feet when speed limit is greater than 30 MPH E. Distance between signs is mandatory unless field conditions determine otherwise. 1. PERMIT IS REQUIRED TO BE VISIBLE AT JOB SITE. 2. Call 253-435-3612 24hrs PRIOR to starting work to open the permit. 3. Call 253-405-1707 to notify the inspector of specific project scheduling. 4. Adhere to the approved traffic control plan. 5. DO NOT open cut without prior approval. 6. DO NOT park vehicles on city sidewalk. Curb, gutter or sidewalk damaged shall be replaced. 7. Call 253-435-3612 to closse the permit when work is complete. 8. RCW 19.122 requires all owners of underground facilities to notify pipeline companies of scheduled excavations through the one-number locator service if proposed excavation is within 100 feet. Notification must occur in a window of not less than 2 business days but not more than 10 business days before beginning the excavation. If a transmission pipeline company is notified that excavation work will occur near a pipeline, a representative of the company must consult with the excavator on-site prior to excavation. 9. I hereby acknowledge that I have read and understand the contents of this permit and I hereby state that the information I have supplied is true and correct. 10. The Applicant shall defend, indemnify, and hold the City, its officers, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the Applicant or on the Applicant's behalf out of issuance of this permit.
