: *****PLEASE READ NEW PERMIT CONDITIONS***** ***SITE SPECIFIC CONDITIONS*** (This may include night work, coordination with inspector prior to work start, school zone restrictions, etc.) THE CITY OF PUYALLUP DOES NOT APPROVE, AUTHORIZE, OR PERMIT WORK OUTSIDE OF CITY LIMITS OR IN THE PIERCE COUNTY RIGHT OF WAY (80TH ST E). CLOSURE ALLOWEDon E PIONEER, 21ST ST SE, & 33RD ST SE: Monday - Friday 10AM - 2PM Call communications center at 253-287-4456 on day of closure. CLOSURE ALLOWED on 8TH AVE SE: Monday - Friday 9AM - 3PM Call communications center at 253-287-4456 on day of closure. Place a SIDEWALK CLOSED, CROSS HERE sign to direct southbound pedestrians on 21ST ST SE to the easterly sidewalk. LEFT LANE CLOSED AHEAD sign required for northbound 21ST ST SE traffic. The applicant shall contact Tamara Ferrin, Puyallup Police Department Support Specialist, at 253-841-5436 or tame@puyallupwa.gov to coordinate traffic support with a uniformed police officer at least 24 hours in advance of work commencement. All aerial crossings at an intersection shall be a maximum of 3 minutes. When shifting traffic into a turn lane, use of double cones is required. No disruption of signal timing allowed at signalized intersection unless the traffic control plan depicts a uniformed police officer at the intersection. All vehicles, equipment, and personnel are prohibited at all times from occupying any portion of the railroad crossing. Railroad crossing signs and striping shall be visible at all times. Vehicle and pedestrian traffic shall not be queued on the railroad crossing. Flaggers shall remain in radio contact at all times. The applicant shall notify property owner(s) of blocked driveway(s) 24 hours in advance of work commencement. No work shall impact the crosswalk area. Work shall cease in the work zone for a pedestrian spotter, who shall be available at all times, to escort pedestrians around the work area, especially at the Foothill Trail. Inspection time performed during a regular 8 hour day Monday - Friday 7:00 am - 5:00 pm is included in the permit fee. Inspection time performed outside of those hours are charged to the permit holder at a rate of $130 per hour with a three hour minimum. GENERAL CONDITIONS: 1. PERMIT AND APPROVED TRAFFIC CONTROL PLAN ARE REQUIRED TO BE AVAILABLE AT JOB SITE. NO PERMIT AT SITE SHALL BE SUBJECT TO TRIPLE PERMIT FEE. 2. Applicant/contractor shall notify the City within 48 hours prior to work start date and also to close the permit when all work and restoration are complete and ready for final inspection. Follow approved Traffic Control Plan (TCP) 4. Follow allowed work hours as seen above. 5. If a lane will be closed, call Comm Center at least 24 hours prior to closure at 253-287-4456. 6. Spotter must be available to assist with pedestrians if needed. 7. To schedule inspection, go to: Secure.cityofpuyallup.org and select appropriate code. 8. Please utilize traffic control information below for TCP: A. Distance between cones equals speed limit in unit feet. B. Taper length equals width of offset times speed limit squared divided by 60. L = WS² /60 - W= lane width S = speed limit C. Sign spacing shall be 100 feet when speed limit is 30 MPH or less D. Sign spacing shall be 350 feet when speed limit is greater than 30 MPH E. Distance between signs is mandatory unless field conditions determine otherwise. 9. DO NOT open cut without prior approval. 10. DO NOT park vehicles on city sidewalk. Curb, gutter or sidewalk damaged shall be replaced. Indemnification / Hold Harmless The Permittee shall defend, indemnify and hold the Public Entity, its officers, officials, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the Permittee or on the Permittee's behalf out of issuance of this Permit, except for injuries and damages caused by the sole negligence of the Public Entity. However, should a court of competent jurisdiction determine that RCW 4.24.115 applies to this Permit, then the Permittee agrees to defend, indemnify and hold the Public Entity, its officers, officials, employees and volunteers harmless to the maximum extent permitted thereunder. It is further specifically and expressly understood that the indemnification provided herein constitutes the Permittee's waiver of immunity under Industrial Insurance, Title 51 RCW, solely for the purposes of this indemnification. This waiver has been mutually negotiated by the parties. The provisions of this section shall survive the expiration or termination of this Agreement.
