: Development Engineering standard right-of-way conditions: Standard Follow approved Traffic Control Plan (TCP). Work between the hours of 9AM & 3PM. Call Comm Center day(s) of lane closure at 253-841-5431. - Inspection time performed during a regular 8 hour day Monday - Friday 7:00 am - 5:00 pm is included in the permit fee. Inspection time performed outside of those hours are charged to the permit holder at a rate of $130 per hour with a three hour minimum. - Distance between cones equals speed limit in unit feet. - Taper length equals width of offset times speed limit squared divided by 60. L = WS² /60 where W= lane width and S = speed limit. - Sign spacing shall be 100 feet when speed limit is 30 MPH or less. - Sign spacing shall be 350 feet when speed limit is greater than 30 MPH. - Distance between signs is mandatory unless field conditions determine otherwise. - Applicant/contractor shall notify the City within 48 hours prior to work start date and also to close the permit when all work and restoration are complete and ready for final inspection. - I agree to have the approved permit on site at all times. - Utility inspections are required prior to backfilling. - No work shall be done in or on the public right of way without a licensed and bonded contractor first obtaining a right of way permit. - I agree to keep the existing right of way free of debris and dirt. - I agree to call the Utility Notification Center at 1-800-424-5555 before beginning any excavation. Call before you dig, it's the law. - I agree to adhere to the approved Traffic Control Plan. - I agree to perform all work in accordance with City's Standards. Any curb, gutter or sidewalk broken now or during construction shall be removed and replaced. - I hereby acknowledge that I have read and understand the contents of this permit and I hereby state that the information I have supplied is true and correct. Indemnification / Hold Harmless: The permittee shall defend, indemnify and hold the public entity, its officers, officials, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the permittee or on the permittee's behalf out of issuance of this permit, except for injuries and damages caused by the sole negligence of the public entity. However, should a court of competent jurisdiction determine that RCW 4.24.115 applies to this permit, then the permittee agrees to defend, indemnify and hold the public entity, its officers, officials, employees and volunteers harmless to the maximum extent permitted thereunder. It is further specifically and expressly understood that the indemnification provided herein constitutes the permittee's waiver of immunity under industrial insurance, Title 51 RCW, solely for the purposes of this indemnification. This waiver has been mutually negotiated by the parties. the provisions of this section shall survive the expiration or termination of this agreement.
Private Utility Permit Conditions Open Engineering Division Standard Conditions
: APPLICANT/CONTRACTOR REQUIREMENTS: 1. PERMIT AND APPROVED TRAFFIC CONTROL PLAN ARE REQUIRED TO BE AVAILABLE AT JOB SITE. NOT HAVING A PERMIT ONSITE SHALL BE SUBJECT TO TRIPLE PERMIT FEE. 2. THE CITY SHALL BE NOTIFIED 24-48 HOURS PRIOR TO JOB START 3. THE CITY SHALL BE NOTIFIED WHEN ALL WORK AND RESTORATION IS COMPLETE AND READY FOR FINAL INSPECTION. PRIOR TO RESTORATION, COORDINATE WITH AN INSPECTOR. *** SITE SPECIFIC CONDITIONS *** CLOSURE ALLOWED: Monday - Friday 9AM - 3PM Call communications center at 253-287-4456 on day of closure. Request a job start inspection through the CityView portal. The City-issued permit packet shall be posted at the job site and available to inspectors at all times. All proposed franchise utilities shall be located within a recorded utility easement or in the public right-of-way as determined by the City of Puyallup. The City of Puyallup does not authorize or permit utility work on private property. Public ROW ends approximately 20 feet behind edge of pavement on 27TH AVE SE and 10 feet behind face of curb on 5TH ST SE. A 10" diameter core drill is allowed for potholing. For pothole windows larger than 10" in diameter or trenching completed in existing roadway, the contractor shall restore the pavement by installing the trench patch and then grinding/overlaying. All roadway restoration and street patches shall comply with City Standard Detail 01.01.20. GENERAL CONDITIONS: 1. Follow approved Traffic Control Plan (TCP) 2.
