: 1- APPOINTMENT REQUIRED FOR ALL INSPECTIONS 2- PROVIDE PLANS AND PERMIT ON SITE FOR INSPECTION. 3- COMPLETE LABOR AND INDUSTRIES INSPECTIONS PRIOR TO REQUESTING CITY INSPECTION. 4- DEVICE MAPS REQUIRED AT THE ANNUNCIATOR AND FIRE ALARM PANEL PRIOR TO CALLING IN FINAL. 5- TOTAL COVERAGE IS REQURED; SUBJECT TO FIELD INSPECTION. COMPLY WITH NFPA 72, 2013 6- FIRE ALARM SYSTEM SHALL BE PLACARDED PER NFPA 72. THE UL CERTIFICATE SHALL BE POSTED AT TIME OF FINAL INSPECTION. THE SYSTEM WILL NEED TO BE UL CERTIFIED. 7- A PRE-TEST SHALL BE CONDUCTED AND A PRINT OUT OF THE LOCATION OF THE DEVICE, POINT NUMBER AND DESCRIPTOR SHALL BE FURNISHED TO THE INSPECTOR AT FINAL. 8- FIRE SPRINKLER TAMPER VALVES AND PIV WILL BE REQUIRED TO BE TESTED AT INSPECTION. FIRE SPRINKLER CONTRACTOR TO BE ONSITE. 9- THE CITY OF PUYALLUP ALLOWS ONE FACP PER BUILDING. THE PANEL WILL NEED TO BE LARGE ENOUGH TO HANDLE MULTIPLE TENTANTS IF APPLICABLE.

: The subject property has been classified as a contaminated site by the Tacoma Pierce County Health Department (TPCHD) and must be remediated per Washington Administrative Code Title 173. It is the applicant’s responsibility to coordinate cleanup with TPCHD. Visit https://www.tpchd.org/healthy-places/contaminated-property-cleanup for more information. The applicant shall request a remediation status report from TPCHD’s Rob Olsen by emailing him at rolsen@tpchd.org. Provide a copy of the status report to the City.
