: APPLICANT/CONTRACTOR REQUIREMENTS: 1. PERMIT AND APPROVED TRAFFIC CONTROL PLAN ARE REQUIRED TO BE AVAILABLE AT JOB SITE. NOT HAVING A PERMIT ONSITE SHALL BE SUBJECT TO TRIPLE PERMIT FEE. 2. THE CITY SHALL BE NOTIFIED 24-48 HOURS PRIOR TO JOB START 3. THE CITY SHALL BE NOTIFIED WHEN ALL WORK AND RESTORATION IS COMPETE AND READY FOR FINAL INSPECTION. PRIOR TO RESTORATION, COORDINATE WITH AN INSPECTOR. *** SITE SPECIFIC CONDITIONS *** ** Work on Pole #48 will be done Sunday night through Thursday night between the hours of 9PM and 5AM only ** ** Work on Pole #57 will be done Sunday night through Thursday night between the hours of 7PM and 5AM only ** ** Work on Pole #48 S and Pole 49 will be done Monday through Thursday 9AM - 2PM only ** ** See permit PRFUP20220082 also for poles 48 & 49 (AKA poles 08 & 09) ** ** The use of double cones is required when routing traffic into a turn lane ** GENERAL CONDITIONS: 1. Follow approved Traffic Control Plan (TCP) 2. Follow allowed work hours see above and shown on each approved TCP 3. If a lane will be closed, call Comm Center on day of closure prior to work start at 253-841-5431. 4. Spotter must be available to assist with pedestrians if needed. 5. To schedule inspection, go to: Secure.cityofpuyallup.org and select appropriate code. 6. Please utilize traffic control information below for TCP: A. Distance between cones equals speed limit in unit feet. B. Taper length equals width of offset times speed limit squared divided by 60. L = WS² /60 - W= lane width S = speed limit C. Sign spacing shall be 100 feet when speed limit is 30 MPH or less D. Sign spacing shall be 350 feet when speed limit is greater than 30 MPH E. Distance between signs is mandatory unless field conditions determine otherwise. 7. Do not open cut without prior approval. 8. Do not park vehicles on city sidewalk. Curb, gutter or sidewalk damaged shall be replaced. Inspection time performed during a regular 8-hour day Monday through Friday 7:00 am - 5:00 pm is included in the permit fee. Inspection time performed outside of those hours are charged to the permit holder at a rate of $130 per hour with a three-hour minimum. INDEMNIFICATION / HOLD HARMLESS: The Permittee shall defend, indemnify and hold the Public Entity, its officers, officials, employees and volunteers harmless from any and all claims, injuries, damages, losses or suits including attorney fees, arising out of or in connection with activities or operations performed by the Permittee or on the Permittee's behalf out of issuance of this Permit, except for injuries and damages caused by the sole negligence of the Public Entity. However, should a court of competent jurisdiction determine that RCW 4.24.115 applies to this Permit, then the Permittee agrees to defend, indemnify and hold the Public Entity, its officers, officials, employees and volunteers harmless to the maximum extent permitted thereunder. It is further specifically and expressly understood that the indemnification provided herein constitutes the Permittee's waiver of immunity under Industrial Insurance, Title 51 RCW, solely for the purposes of this indemnification. This waiver has been mutually negotiated by the parties. The provisions of this section shall survive the expiration or termination of this Agreement.
Pole Placement Open Development & Permitting Services Standard Conditions
: Where a curb exists, the lateral offset for all vertical obstructions shall be a minimum of 1.5' from the face of the curb to the face of the vertical obstruction and a minimum of 3' at intersections. Where no curb exists, the lateral offset for all vertical obstructions shall be a minimum of 4' measured from the face of the obstruction to the edge of pavement.
Police Department Traffic Control Resolved Development & Permitting Services Standard Conditions
: For approved Traffic Control Plans that necessitate the use of police officer assistance, Puyallup police officers will be utilized. Contact Tamara Ferrin at 253-841-5436 or via email at tame@puyallupwa.gov to coordinate. 24 hrs advanced notice is required for coordination/scheduling purposes.
Night Traffic Control Overtime Inspection Fee Open Development & Permitting Services Standard Conditions
: Whenever work is progressing within the city, inspectors are required to be available for inspection. Therefore, work beyond a regular 8 hour day also needs inspected. If your project is the only one operating at that time, you will be required to pay for the inspector to be there full time (3 hour minimum call out time). Whenever possible, the city will distribute fees among other projects working at the same time.
Night Work Conditions Open Development & Permitting Services Standard Conditions
: NIGHT WORK REQUIREMENTS: APPLICANT MUST PROVIDE 48 HOUR NOTIFICATION TO RESIDENTS WITHIN 300 FEET OF THE WORK AREA EITHER BY DOOR HANGER, MESSAGE BOARD OR SIGNS POSTED VISIBLY ALONG THE RIGHT-OF-WAY WORK ALLOWED SUNDAY NIGHT THROUGH THURSDAY NIGHT ONLY BETWEEN THE HOURS OF 9PM AND 5AM 48-HOUR ADVANCE NOTICE IS REQUIRED FOR COORDINATION/SCHEDULING.
